How to apply for PM-JAY Ayushman Bharat Health Card

To apply for the PM-JAY (Pradhan Mantri Jan Arogya Yojana) Ayushman Bharat health card, you can follow these steps:

1. Eligibility Check: Ensure that you meet the eligibility criteria for the PM-JAY scheme. The scheme primarily targets economically vulnerable families. These are the people whose name appears on SECC-2011 data and some other special category. You can check the eligibility criteria on the official PM-JAY website or inquire at a nearby Common Service Center (CSC) or empaneled hospital. Alternatively, you can visit our Centre, Kruz Digital at Namdilong Kanglatongbi, Near Chil Chil Asian Mission School. 

2. Document Preparation: Once you made sure your eligibility - 

(a) collect a Form from our Centre. The form is to be filled up and signed by Village head/ Chairman or any concerned Authority.

(b) You can also bring the Ayushman Bharat letter received from Govt of India through India Post.

(c). Bring one document proof i. e, Aadhaar card. The specific documents required can vary depending on the state or union territory where you are applying.

3. Registration: The operator will initiate registration process once your eligibility and document is verified. You will be authenticated by scanning your fingerprint or iris,for this your physical presence is mandated. 

4. Application Submission: After you are biometrically authenticated, the operator will submit the form by uploading all the necessary document.

5. Verification: Your details and documents uploaded will be verified by the concerned authorities. The verification may be instant or take upto 1 week or more.. .

6. Issuance of Health Card: Once your application is approved, you will receive the PM-JAY Ayushman Bharat health card. You are advised to collect the card from where you have applied. This card will serve as your proof of enrollment in the scheme. It will contain your personal details and a unique identification number that you can use to avail of healthcare services at empaneled hospitals.
